We would like to set up a Patient Participation Group and are asking those who may be interested, to take a few moments to complete the following demographic and interest survey.

To ensure that we have good representation of the practice patients within the group, the questions are designed to focus on specific areas.

At this stage we do not know how many patients we require. We anticipate that meetings will be held quarterly and are keen to appoint a Chairperson to carry the group forward with our support.
